October is a vital month for global health, dedicated to raising awareness about liver cancer, specifically Hepatocellular Carcinoma (HCC). At Inova Hospital Kathmandu, we are committed to leading the fight against this disease in Nepal through advanced care, prevention, and education. The key message is clear: Liver Cancer Does Not Discriminate, but Early Detection Saves Lives.
Understanding Your Liver: The Body’s Silent Factory
The liver is your body’s largest internal organ, performing over 500 essential functions. It has a remarkable ability to regenerate. However, this resilience has its limits. A liver damaged by cirrhosis often loses its regenerative capacity, increasing the risk of cancer.
Are You at Risk for Liver Cancer in Nepal?
Knowing your risk is the first step toward prevention. The most common risk factors we see at our liver clinic in Kathmandu include:
- Chronic Viral Hepatitis: Hepatitis B and C are leading causes of liver cancer worldwide, including in Nepal.
- Cirrhosis: Any cause of chronic liver scarring significantly increases risk.
- Excessive Alcohol Consumption: Alcohol is a direct liver toxin and carcinogen.
- Metabolic Conditions: Obesity, Type 2 Diabetes, and Fatty Liver Disease (NAFLD/NASH) are rising risk factors.
- Family History: A family history of liver cancer can increase your risk.
- Dietary Factors: Exposure to aflatoxins (found in improperly stored grains and nuts) is a known risk.
Prevention and Early Detection: Your Best Defense
The good news is that liver cancer is largely preventable. As a liver transplant surgeon, I emphasize that preventing the underlying liver disease is the most effective strategy.
1. Primary Prevention: Stop Liver Disease Before It Starts
- Get Vaccinated: The Hepatitis B vaccine is over 95% effective and is the first-line defense against HBV-related liver cancer.
- Treat Viral Hepatitis: Modern medicines can effectively control Hepatitis B and cure Hepatitis C, drastically reducing your cancer risk.
- Limit Alcohol: There is no “safe” level of alcohol for cancer prevention. Reducing intake is crucial.
- Live Healthily: A balanced diet and regular exercise help prevent fatty liver disease and diabetes.
2. Secondary Prevention: The Critical Role of Screening
For those with existing liver conditions, regular screening is non-negotiable. At Inova Hospital, we recommend:
All patients with Liver Cirrhosis or chronic Hepatitis B & C must undergo an abdominal ultrasound and AFP blood test every 6 months.
This simple, cost-effective protocol allows us to detect cancer at its earliest, most treatable stages.
Advanced Diagnosis and Treatment at Inova Hospital Kathmandu
When a liver mass is suspected, our state-of-the-art radiology department utilizes triphasic CT scans and dynamic MRI for highly accurate, non-invasive diagnosis.
Our multidisciplinary team of HPB surgeons, hepatologists, and oncologists provides comprehensive, personalized care. Treatment options we offer include:
- Liver Resection: Surgical removal of the tumor, ideal for patients with localized cancer and a healthy liver.
- Liver Transplant: This is the best option for patients whose cancer is confined to the liver but have underlying cirrhosis. At Inova Hospital, we assess patients using international criteria (like Milan Criteria) for transplant eligibility, which offers an excellent chance of long-term survival and cure.
- Locoregional Therapies: For patients awaiting transplant or who are not surgical candidates, we offer advanced procedures like Radiofrequency Ablation (RFA), Trans-Arterial Chemoembolization (TACE), and Trans-Arterial Radioembolization (TARE).
- Systemic Therapy: We provide access to the latest targeted chemotherapy and immunotherapy drugs for advanced cancers.
